By completing and signing this form, you understand that you are surrendering your rights to this animal to the tagging rescue coordinated by MM ARC. Once transport has taken place, you are NOT able to retain the animal or retrieve the animal back without following the application process of the rescue that obtained ownership of the animal through the coordination of MM ARC.
By completing and signing this form, you are giving MM ARC permission to network or assist in private rehoming for your pet. Transactions of the subject animal after the signing of this form are final.
MM ARC does not retain liability for the ultimate outcome of the subject animal once transferred to the tagging rescue. All grievences regarding animals transferred to rescue must discussed with the rescue group directly.
